• To earn a $75 monthly credit toward your healthcare premiums, log in to Health Advocate.
• You must complete your Biometric Health Screening and Personal Health Profile within 90 days from your eligibility date (benefits effective date).

How to Complete Your Biometric Health Screening

Go to LabCorp facility

• Log in at Healthadvocate.com/Azuria .
• Click Complete Your Health Screening on the homepage To Do list.
• Click Find a LabCorp Location.
• Click Find Location.
• Click Print Voucher in the upper right corner.
• Follow the instructions to find a participating LabCorp.
• Schedule your appointment.
• Be sure to bring your voucher and photo ID.
• LabCorp will send the results to Health Advocate.

Go to your doctor

• Log in at Healthadvocate.com/Azuria .
• Click Complete your Health Screening on the homepage To Do list.
• Click Download Physician Form.
• Click Download.
• Print the PDF form and take to your physician to complete.
• Your physician will send the completed form to Health Advocate.
